Worker's Compensation

When a job related injury occurs, the injured worker is asked to find a specialist to evaluate and treat their condition for worker's compensation. Patients receiving worker's compensation benefits may undergo evaluation by the employer's doctor, or by a community physician. It is important to choose an orthopedic specialist that has not only the experience but also the integrity when providing worker's compensation treatment. 

Early diagnosis and treatment of musculoskeletal injuries are the key to returning an injured worker to their job as soon as possible. Most worker's compensation claims are orthopedic in nature and require comprehensive, personalized treatment plan. As an experienced hand and upper extremity specialist, I understand the special challenges presented by industrial injuries and injured worker's needs. I believe in working closely with my patients to develop appropriate work restrictions.

2.    Contact Dr. Izadi's worker's compensation liason, Jan Kruse, at (402) 354-0760 and provide her with: 

• Your case worker's name and phone number
  
• Your supervisor's name and phone number

3.    Print and fill out the following forms:                                 

• Worker's compensation.pdf
  
• History and Physical.pdf 
  
• Cerner Patient Registration Information.pdf
  
• Privacy Notice.pdf       

4.    Any medical records pertaining to your current condition
5.    Any studies (X-ray, MRI, CT, nerve studies) with reports pertaining to your current condition 
6.    Valid ID
